You can fax the order form back, or call in your order. Whatever is most convenient for you.

Step Three:
First order is C.O.D. or credit card (Visa, M/C, Amex) or prepaid. When prepaying please add 5% inside California, or 10% outside California for shipping and handling. If paying by check, please allow 10 days for clearing.
